ProVision Athletics 2026 Summer Baseball Camps Waiver and Release of Liability

Summer Camp Participation

I, the undersigned, am the parent or legal guardian of the above-named minor participant and have the legal authority to sign this agreement on their behalf. I understand that the participant is voluntarily enrolling in and participating in one or more sessions of the 2026 Summer Baseball Camps hosted by ProVision Athletics (the “Facility”). This agreement shall apply to all weeks, sessions, and dates the participant attends during the 2026 summer season. Camp activities may include, but are not limited to: batting, fielding, throwing, pitching machine use, base running, agility drills, strength and conditioning exercises, scrimmages, and other baseball and athletic training activities conducted on or off-site.

Assumption of Risk

I acknowledge and understand that participation in athletic training and sports activities involves inherent risks, including but not limited to falls, collisions, contact with equipment, overexertion, property damage, serious bodily injury, permanent disability, concussion, or death. I knowingly and voluntarily assume all risks associated with participation in the 2026 Summer Baseball Camps for my minor child, or any child for whom I am legally responsible.

Release and Waiver of Liability

In consideration for being permitted to participate in the 2026 Summer Baseball Camps, I, on behalf of my minor child, or any child for whom I am legally responsible hereby release, waive, discharge, and hold harmless ProVision Athletics, its owners, directors, employees, coaches, staff members, independent contractors, volunteers, agents, and representatives from any and all claims, demands, liabilities, damages, or causes of action arising out of or related to participation in camp activities, including claims arising from the negligence of the Facility or its representatives, to the fullest extent permitted by law. I further agree to indemnify, defend, and hold harmless ProVision Athletics from any claims, damages, liabilities, costs, or attorney’s fees arising from participation in the camp or any breach of this agreement.

Medical Acknowledgment and Emergency Authorization

I certify that the participant is physically fit, in good health, and able to safely participate in camp activities. I agree to disclose any relevant medical conditions, allergies, injuries, or limitationsprior to participation. In the event of illness or injury, I authorize ProVision Athletics staff to obtain emergency medical treatment for the participant if I cannot be reached. I understand and agree that I am financially responsible for any medical care provided.

Insurance Responsibility

I understand that ProVision Athletics does not provide medical or accident insurance coverage for camp participants. I acknowledge that it is my responsibility to maintain adequate health and accident insurance coverage for the participant.

Camp Rules and Conduct

I agree that the participant will follow all camp rules, safety guidelines, and staff instructions. I understand that failure to comply with camp policies or engaging in unsafe, disruptive, or inappropriate behaviour may result in dismissal from the 2026 Summer Baseball Camps without refund.

Media Release and Consent (Required Condition of Participation)

As a material condition of participation in the 2026 Summer Baseball Camps, I grant ProVision Athletics full permission to photograph, video record, or otherwise capture the participant’s image, likeness, voice, or performance during camp activities. I authorize ProVision Athletics to use, reproduce, publish, distribute, and display such media for lawful business purposes, including but not limited to marketing, advertising, promotional materials, website content, social media, print materials, and educational use, in any medium now known or later developed, without compensation. I understand and agree that this Media Release is a mandatory condition of participation. I waive any right to inspect or approve the final media product and release ProVision Athletics, its owners, employees, contractors, agents, and representatives from any claims arising out of the use of such media, including claims related to privacy, publicity, or defamation.

Acknowledgment and Agreement

By signing below, I acknowledge that I have read and fully understand this 2026 Summer Baseball Camps Waiver and Release of Liability, including the required Media Release and Consent section and the Indemnification clause. I understand that signing this document is required for participation and that it is legally binding. I further acknowledge that if any provision of this agreement is found unenforceable, the remaining provisions shall remain in full force and effect.
