PHOTO & VIDEO RELEASE – OCTOBER 14, 2026

ProVision Athletics will be hosting a professional photography session on October 14, 2026.

During this session, photographs and/or video may be taken of athletes participating in ProVision Athletics practice and training.

By signing below, I give permission for my child to be photographed and/or recorded during the October 14 session and authorize ProVision Athletics and its authorized third-party photographers, to photograph my child.

I also authorize ProVision Athletics and its authorized third parties to use, reproduce, publish, display, and distribute selected photographs for promotional, marketing, portfolio, advertising, and informational purposes, including:

• ProVision Athletics website

• Third-party photographer or media provider websites and portfolios

• Social media accounts

• Digital and print advertising

• Promotional videos and materials

• Email communications and newsletters

• Program, camp, clinic, team, and travel ball promotion

• Other ProVision Athletics promotional and marketing materials

I understand that photographs published online or through social media may be viewed, shared, or reproduced by others outside the control of ProVision Athletics.

I understand that no compensation will be provided for the use of these photographs and that the images may be edited, reproduced, published, or distributed for the purposes outlined above.

ProVision Athletics will not intentionally identify a minor athlete by full name in promotional materials without

additional permission from the athlete's parent or guardian.

By signing below, I confirm that I am the parent or legal guardian of the athlete named above and that I have read and

agree to the terms of this Photo Release.
